Terms and Conditions
Eligibility:
- This program is a dental savings discount plan, not dental insurance.
- This plan can only be used at Exceptional Dental of St. Petersburg.
- This plan cannot be combined with any other dental insurance plan or CareCredit.
- This plan cannot be combined with any other coupons, discounts, or special offers.
- All patients are subject to Exceptional Dental of St. Petersburg office policies.
Payments:
- Membership fee is due at time of enrollment.
- All payments for treatments must be paid in full at the time of service to receive a discount. Any services that are not paid in full at the time of service will be billed at our regular fees.
- All payments are non-refundable.
- No refunds will be given if a member does not use the plan benefits, relocates, or obtains dental insurance.
- 12 months term begins the day you sign-up
Exclusions:
- Plans and fees are subject to change yearly.
- No discount is provided for services requiring referral to a specialist.
- Should treatment be needed following an injury or 3rd party outside insurance is involved, this discount cannot be used.
- Treatment initiated prior to enrollment is not eligible for discounts.
- Treatment fees are guaranteed for 180 days from the date quoted by the office.
- Dental products are not included.
